Abstract

A man in his 70s was brought to our hospital on an emergency basis with chief complaints of fever, cough, and diarrhea. Abdominal computed tomography (CT) showed appendicitis accompanied by an abscess, as well as venous thrombosis extending from the ileocolic vein to the superior mesenteric vein (SMV), and SMV thrombosis was diagnosed. Ileocecal resection and lavage and drainage were performed on the same day for the appendicitis, and anticoagulant therapy and treatment with antibiotics were given postoperatively for the SMV thrombosis. The patient's postoperative course was uneventful, and the patient was discharged on day 17 when warfarin control was stabilized. Oral administration of warfarin was discontinued after confirming the disappearance of thrombi on an outpatient basis. The number of reported cases of SMV thrombosis complicating appendicitis in Japan is relatively low (n=9), and increases in severity have been reported in some cases. The present case is reported in the context of the relevant literature.
