A 72-year-old man was admitted to our hospital because he had undergone weight loss. He had previously undergone laparotomy for the treatment of duodenal ulcer. Abdominal computed tomography revealed granular calcification and a tumor (2.5 cm × 3.0 cm) showing a contrast effect at the front of the pancreas. Fluorodeoxy glucose-positron emission tomography revealed abnormal uptake in the same location. Laparotomy was performed ; the patient had previously undergone pyloric exclusion. The pyloric ring and gastric pyloric antrum were left intact, and the tumor was detected in the remnant stomach. The remnant stomach was resected. The pathological findings indicated papillary adenocarcinoma. Here, we report a rare case of distal gastric remnant cancer after pyloric exclusion.
