Abstract

The patient was a 1-year, 10-month-old boy with an anocutaneous fistula, which was treated by perineal anoplasty (cut-back operation). After the operation, the child developed an abdominal bulge in the right flank, and had constipation. A barium enema showed segmental dilatation of the large intestinal segment extending from the rectum to the sigmoid colon. Rectal biopsy showed no hyperplasia of the Ach-E(+) nerve plexus. We performed resection of the dilated bowel segment and endorectal pull-through of the normal colon. After the operation, the child became able to defecate and the constipation resolved. Segmental dilatation of the colon is often the cause of chronic constipation. Surgical treatment could be expected to resolve the persistent symptom.
