- 腹腔動脈領域の非破裂segmental arterial mediolysisの1例

Description
Recently, segmental arterial mediolysis (SAM), which can result in intraabdominal ruptured aneurysm, has attracted attention. Many cases of SAM are diagnosed following rupture. However, in this reported patient, the aneurysms associated with SAM could be resected in their non-ruptured state. A 35-year-old woman was found to have an epigastric tumor in June 2008. Abdominal ultrasonography and CT showed two aneurysms (5 cm and 2 cm in diameter) located close to the pancreatic head. Embolization of the artery connected to the aneurysms was not feasible because of the large size of the aneurysms and their location ; therefore, a resection was planned. The 5 cm in diameter aneurysm was located at the divergence of the gastroduodenal and the right gastroepiploic arteries, and the 2 com in diameter aneurysm was located close to the 5 cm in diameter aneurysm and involved the anterior superior pancreatico-duodenal artery. On pathology, the resected specimen showed bubble degeneration of the arterial media ; SAM was diagnosed. The pathogenesis of SAM is unclear, and recurrence has been reported. Therefore, close follow-up is warranted in patients with resected aneurysms associated with.
