Mucinous Cystadenocarcinoma of the Appendix that was Difficult to Differentiate from an Abscess Originating from Appendicitis

A 71-year-old woman was admitted to our hospital with right lower quadrant abdominal pain. Abdominal computed tomography showed an abscess encircled by the cecum and sigmoid colon. The diagnosis was an abscess originating from appendicitis, and conservative therapy was given. Her abdominal pain and inflammatory reaction improved after conservative therapy. However, the abscess did not shrink, and the inflammatory reaction did not normalize ; therefore, she was finally diagnosed with carcinoma of the appendix, and surgery was planned. During the operation, the appendix could not be identified, and the cecum adhered rigidly to the ileum, ascending colon, sigmoid colon, bladder, and right ovary. These areas were successfully resected without mucus dissemination, and histological examination showed well-differentiated mucinous cystadenocarcinoma at the site of the appendix.<BR>Confirming the diagnosis in this case that initially appeared to be an abscess derived from appendicitis was difficult. It is important to obtain a preoperative diagnosis and select the appropriate surgical treatment in such cases.
