A 48-year-old man was admitted to our hospital with jaundice. Abdominal CT and MRI showed gallbladder stones impacted in the hilar bile duct. Under the diagnosis of Mirizzi syndrome, open cholecystectomy was performed after placing a biliary drain. We removed the stone in the gallbladder. Another stone was found to be buried in the liver bed. Bile began to spout from the fistula between the gallbladder and the right main hepatic duct after this stone was removed. We performed fistula closure with the residual gallbladder wall and a cystic plate, and placed a T-tube in the common bile duct. There were no postoperative complications. In our case, we obtained good results from a low-stress simple operation for an internal biliary fistula formed between the gallbladder and a branch of the bile duct originating from the common heaptic duct.
