A case of perianal giant-condyloma acuminatum, which was completely resected preserving the anus was reported. A 56-year-old male first visited the Department of Dermatology with a 3-month history of painful bleeding from a rapidly growing perianal tumor. He was treated by cryosurgery and 5-FU ointment for 2 months without any improvement. Since the tumor became larger, he consulted our department for removal of the tumor. Physical examination revealed a 12×7.5-cros infected, friable, cauliflower-like tumor surrounding the anal orifice, but the tumor did not extend to the anal canal. The diagnosis of perianal giant condyloma acuminatum without malignancy was made by histological examination of the tumor. The tumor, including normal skin with a 5-mm safety margin from the tumor, was removed and the anal canal was preserved. The skin resulting defect was sutured, and an anastomosis was made between the skin and anal canal. Finally a covering colostomy with the transverse colon was made to protect the anastomosis. Microscopic study of the excised specimen revealed marked papillomatosis and acanthosis of the epidermal cells without malignancy. The operative wound healed very well. Three months later, the temporary colostomy was closed. The patient has done well without any complaint for 18 months after surgery.
