From 1985 to 2003 we experienced twelve cases of primary carcinoma of the vermiform appendix, which accounted for 0.69%of all resected colorectal carcinomas in our hospital. Of the twelve cases, three cases were male and nine cases were female, with the average of 56.8 years ranging from 33 to 74. The chief complaints included right lower abdominal pain, palpable abdominal mass, and high fever, though two asymptomatic cases were found by anemia and stool occult blood in health checks. Preoperative diagnosis was cecal cancer in three cases, peritonitis cartinomatosa in three, intra-abdominal abscess of the ileocecal region in one, and appendiceal tumor in only three. We performed right hemicolectomy in 6 cases, ileocecal resection in 4, and appendectomy in one. Regarding clinicopathological staging, 3 cases were in stage II, one case stage IIIa, and 8 cases stage IV. The stage IV cases included peritoneal dissemination in 6 and liver metastasis in 3. Four cases remained alive, but 8 cases died soon after the operation. Our experience suggests that primary carcinoma of the appendix should be considered as a probable differential diagnosis in patients with lower abdominal pain or an abdominal tumor.
