Laparoscopic Lower Anterior Resection for a Patient with Situs Inversus Totalis

Abstract

A 48-year-old man had been diagnosed in infancy with situs invertus totalis. In a routine medical checkup a fecal occult blood test was positive, and a subsequent colonoscopy revealed a 0-IIa+IIc lesion 20 mm in size on the posterior wall of the lower rectum (Rb), which was diagnosed by biopsy as well-differentiated adenocarcinoma. Carcinoma with mild submucosal invasion was suspected on the basis of ultrasound endoscopy, and endoscopic submucosal dissection was therefore performed. Histopathological examination, however, revealed carcinoma with severe submucosal invasion.<BR>Further investigation did not identify any lymph gland or distant metastasis, and the decision was made to perform laparoscopic lower anterior resection. Although our team had no experience of performing laparoscopic surgery on a patient with situs invertus totalis, the use of three-dimensional computed tomography (3D-CT) angiography to map blood vessels and image training by the left-right inversion of surgical videos enabled the operation to be performed safely.<BR>There have been few reports of laparoscopic rectal cancer surgery in patients with situs invertus totalis, and we therefore report this case together with a brief discussion of the literature.
