Immune complexes (IC) were detected in 70 serum specimens from 10 patients with acute poststreptococcal glomerulonephritis (AGN) by measurement of 125I-C1q binding activity (C1q-BA). The mean value of C1q-BA was significantly increased in AGN (N=70) compared to normal (N=20). In serial measurements (2 to 14 months of the follow-up period), many of the patients (7/10) showed high C1q-BA in the beginning, then it entered into normal range. Additional one patient also had persistently high Clq-BA during 8 weeks of the follow-up period. In general C1q-BA moved in parallel with serum IgG level and ASO titer. It offten reversely correlated to serum CH50, C3 and C4, whereas not correlated to serum C1q. There were no relation between C1q-BA and proteinuria, nor the fixation of immunoglobulins and complement components in the glomeruli. In sucrose density gradient analysis of 3 sera from the 3 patients on day 15, 25 and 27 after the onset, the major portion of the increased C1q-BA was found in macromolecular zone. The present data suggest that the C1q-reactive materials detected by Clq-BT may essentially represent the CIC, which appeared particularly in the beginning of the disease. And they may be continued to be present since the latent phase of AGN.
