抄録

Thirty hypertensive patients whose creatinine clearance were less than 30 ml/min treated with Furosemide combined with other hypotensive agents. <BR>The following conclusions were obtained: <BR>1. Systolic blood pressure dropped more than 30 mmHg in 70% of patients. The effect of lowering blood pressure appeared an average of 15 days after Furosemide had been given. Mean lowering of blood pressure was 57.4 mmlg in systolic and 30.5 mmlg in diastolic respectively. <BR>2. Diuretic effect of Furosemide was prominent among the cases whose lowering blood pressure effect was remarkable. However, this diuretic effect was not clear among the cases whose creatinine clearance was less than 3 ml/min. In cases, whose creatinine clearance was 5-10 ml/min, 160-240 mg of Furosemide daily was adequate in general. In cases of chronic renal failure due to diabetic raephropathy, the hypotensive effect was less prominent. <BR>3. The amount of additive hypotensive agents required, diminished with administration of FurosemideR Propranolol made a favarable effect to decrease blood pressure in cases whose PRAA was high. <BR>4. Some of those cases developed hypokalemia, hyponatremia and hypochloremia as adverse effects, but no cases indicated that Furosemide should be discontinued because of the above side effects.
