Ophthalmology belongs to a physical department which deals with a highly developed sensory system. In the case of patients with irreversible condition, we must consider how to care for their fear of blindness. Psychosomatic care is essential, in ophthalmology as well as in other physical departments, when daily life and mental activity are impaired due to low vision and/or prolonged eye irritation. Nevertheless, in the ophthalmological field, the psychosomatic approach has scarcely been conducted. Since we set up the Japan Psychomedical Ophthalmology Society in 2007, various psychological approaches have been tried. In this manuscript, I introduce clinical manifestations of essential blepharospasm and our psychomedical approaches in the neuro-ophthalmology division of Inouye Eye Hospital. Thirty patietns who were referred to psychiatric departments or mental clinics were identified for the past 3 years. Their most frequent diagnosis was pain disorder.
