Abstract
Before dealing with the theme of this symposium, the author refers to Andreasen's indication about the current situation in the field of mental health, that young psychiatrists in America are not free from the symptomatology mentioned in DSM, and not taught about careful clinical evaluation that is targeted to the individual person's problems and social context and enriched by a good general knowledge of psychopathology. The author emphasizes that Japan is also facing a crisis in the current social situation where evidence and objectivity are taken so seriously that a considerable number of psychiatrists and psychosomatists assess only the disorders, but not the patients. They also only evaluate disorders from patients' outside, lacking in the viewpoint to understand their experiences from inside. On the basis of clinical materials, the auther discusses that the medical mental health field in Japan will change its quailty and there is a possibility that the society may change a lot, if the professionals, such as doctors, clinical psychologists and caseworkers, share a psychoanalytic standpoint as the base of clinical identity through a training, by which to trace and understand patients' experiences from their inside that are paid little attention today.
