A dosage nomogram for trimetaphan to control the blood pressure during ketamine anesthesia

Abstract

Trimetaphan was used to control the blood pressure in 38 patients varying in age from 5-81 years under ketamine, pentazocine & diazepam micro drip infusion or NLA. Blood pressure were obtained with the usual Riva-Rocci sphygmomanometer and recorded every five minute during surgery. Systolic blood pressure was reduced with a rate of less than -Δ3mmHg/min by infusing 0.01-0.025% solution of trimetaphan in vactated Ringer's solution. Blood pressure was maintained to a resting level during anesthesia. An inverse relationship was found between patient age and minute dosage of trimetaphan-induction dosage, and also the minute dosage of maintenance. Previously trimetaphan dosage and concentration was adjusted according to recommended dosage raning from 500-1000 μg/ml. But to eliminate excessive hypotension and other dangerous complication, and to attain consistently safe levels of blood pressure in aged patients, infusion of diluted trimetaphan solution (100-200 μg/ml) has been proved to be very useful.
