Background : The ketone body 3-hydroxybutyrate (3-OHB) is a major intestinal fuel which suppresses bacterial translocation (BT) following burn injury in rats. Enteral fiber also has a suppressive effect on BT. The objective of this study is to evaluate the synergistic effect of combined 3-OHB and soluble fiber administration on the suppression of BT following burn injury. Materials and Methods : Thirty-seven male Wistar rats were subjected to a 20% total body surface area. full thickness scald burn under pentobarbital anesthesia. The animals were divided into five groups, control, glucose, ketone body, fiber, and ketone body and fiber. The substances, including 4.9% 3-OHB and soluble fiber (pectin and guar gum : 0.6g/kg/day) were administrated by gavage every 12 hours. After 72 hours, the mesenteric lymph nodes were harvested for quantitative culture. Results : The quantitative bacterial counts in the ketone, fiber, and ketone/fiber groups, were significantly lower than those in the control and glucose groups (p<.05). The counts in the ketone/fiber group were not significantly different from those in the ketone group. Both groups had significantly lower quantitative counts compared to the fiber group (p<.05). Conclusion : Ketone body administration into the alimentary tract effectively suppresses BT. The combination of fiber and ketone body does not further suppress BT.
