We studied the effect of glucose intolerance on the occurrence of stroke in a prospective population survey which has been conducted in Hisayama town since 1961. At the initial screening, 1621 subjects aged 40 or over (about 90% of the total population) participated in the study. The prevalence of glucose intolerance, estimated by the combination of glycosuria with oral glucose or meal loading test, was 8.0%(130 out of 1621). During 22 years of follow-up, 292 first-time strokes occurred. Risks for all types of stroke and cerebral infarction were conspicuously greater in subjects with glucose intolerance than in those without, for both sexes, after controlling for age. Glucose intolerance significantly affected the risk of cerebral infarction for females even when eight risk variables for cardiovascular disease were controlled (relative risk, 2.71). Hypertension was also an important risk factor for cerebral infarction for females in this study. The cumulative incidence of cerebral infarction for females was significantly higher in hypertensives with glucose intolerance than in those without. We conclude that glucose intolerance is a risk factor for cerebral infarction, especially in females.
