Strict Blood Pressure Control Reduces Urinary Protein Excretion in Patients with Overt Diabetic Nephropathy

Abstract
We investigated whether strict blood pressure control could reduce the urinary protein excretion in 20 diabetic outpatients with overt diabetic nephropathy. The mean systolic blood pressure decreased significantly (p<0.01) from 166.7±20.0 mmHg to 119.9±14.2 mmHg (mean±SD), while the mean diastolic blood pressure also significantly descreased from 79.3±10.2 mmHg to 65.2±15.1 mmHg (mean±SD) after 6 months of a strict antihypertensive treatment regimen. The number of antihypertensive drugs per patient increased from 4.9 to 2.1. The urinary protein-creatinine ratio (U-TP/Ucr), which reflects the daily urinary protein excretion, declined significantly (p<0.01) to 1.20±1.36 g/gCr (mean±SD) from 2.72±2.20 g/gCr. A significant correlation was noted between the degree of blood pressure control and the U-TP/Ucr, although no correlation was noted between improvement of the HbA1c and the U-TP/Ucr. Thus, strict blood pressure control could reduce urinary protein excretion even in patients with overt diabetic nephropathy.
