Effectiveness of Step-up Bolus Insulin Addition on Combination Therapy with Sulfonylurea and Once-daily Insulin Glargine in Japanese Type 2 Diabetic Patients (JUN-LAN Study 7)

We have evaluated the effectiveness of step-up bolus insulin addition in 16 type 2 diabetic patients with a mean HbA1c of 8.12±0.92% who underwent combination therapy with sulfonylurea and once-daily insulin glargine (bedtime or morning) but whose HbA1c was ≥7.0% over 1 year.<br>Patients underwent randomly added bolus insulin before breakfast (n=8) or dinner (n=8). After 12 weeks of treatment, 6 whose HbA1c was ≥7.0% and who agreed to step-up addition of rapid-acting insulin underwent this therapy. After 24 weeks treatment, mean HbA1c in all patients improved from 8.12±0.92% to 7.19±0.54%. Body weight increased slightly but significantly during the study.<br>These results suggest that step-up bolus insulin addition during combination therapy with sulfonylurea and once-daily glargine is effective in lowering HbA1c and easily tolerated in patients having difficulty in undergoing intensive insulin therapy.
