Effects of Smoking on HbA<sub>1</sub>c and Fasting Plasma Glucose

Abstract

We reported recently that the effect of smoking on the fasting plasma glucose (FPG) was observed only in obese people who smoked 40 or more cigarettes per day. In this paper, we report the results of a survey of 2,925 male employees conducted to determine the effect of smoking on the HbA1c as compared with that on the FPG. Multiple logistic regression analysis demonstrated that the odds ratio of subjects with a HbA1c value of 5.5% or more was 1.49 (p<0.05)among smokers of 20 or more cigarettes per day and 1.92 (p<0.1) among those who smoked 40 or more cigarettes per day. When the subjects were divided into two groups according to their body mass index (BMI), the effect of smoking was observed only among subjects with a BMI of less than 24. In contrast, the odds ratio of subjects with a FPG of 110 mg/dl or more was 2.98 (p<0.05) among smokers of 40 or more cigarettes per day and this effect of smoking was observed only among subjects with a BMI of 24 or more. These findings suggest that smoking affects the postprandial plasma glucose to a greater degree than the FPG and that the effect of smoking on the postprandial plasma glucose is more pronounced in nonobese subjects than in obese subjects.
