Intrabiliary Growth of Metastatic Adenocarcinoma 11 Years after Curative Operation of Colon Cancer

Description

A 76-year old man with a history of sigmoidectomy for sigmoid colon cancer 11 years ago was referred to our hospital with deterioration of liver function. CT showed intrabiliary tumor in the right anterior hepatic duct. We diagnosed a intraductal growth type of intrahepatic cholangiocarcinoma, and performed the right hemihepatectomy with extrahepatic bile duct resection after right portal vein embolization. Histological findings and an immunohistochemical examination of CK7, CK20 and CDX2 revealed metastasis from the colon cancer. The tumor extended predominantly along the right anterior hepatic duct without forming an extrabiliary mass. We finally diagnosed an intrahepatic bile duct metastasis from colonic adenocarcinoma.
