We reviwed 18 patients of tibil plafond fractures treated in the last 10 years. Osteoarthritis occurred in 8 cases of fair and poor reduction cases. Ruedi's type 3 fractures were frequently associated with secondary osteoarthritis in spite of anatomical radiographic reduction. Rigid fixation of the fibul was an important factor for prevention of osteoarthritis. It is important to grasp the condition of articular surface of the ankle joint by tomography and CT, and to obtain real anatomicalreduction.
