Increasing complaints of respiratory symptoms have been observed among the Yokkaichi residents since 1962 following the large scale construction of Petroleum Refining Industries in 1960.<br>This study was carried out to clarify the clinical features and prognosis of the patients due to air pollution in this area.<br>1) Since the establishment of the relief institution for these patients (1965), the number of cases officially designated as the patients due to air pollution (designated patients) reached up to 1268 (male: 696, female: 572) until 1972.<br>Of age distribution, the number of cases younger than 9 years old was 502 (39.6%) and that of cases older than 50 years old with a peak at the 7th decade was 463 (36.6%).<br>2) These cases were classified into the following 4 groups according to their clinical diagnosis as: 1) 466 cases of bronchial asthma (36.8%) 2) 448 cases of chronic bronchitis (35.3%) 3) 327 cases of asthmatic bronchitis (25.8%) 4) 27 cases of pulmonary emphysema (2.1%)<br>3) The number of cases cured or cancelled the designation was 159 (20.9%) of 762 cases until 1969 and 72 cases of them (45.3%) were younger than 9 years old.<br>4) The number of death was counted 75 of 1268 cases until 1972 and 80% of them were older than 60 years old.<br>5) The causes of death were respiratory diseases (28 cases: 37.3%), heart diseases (8 cases: 10.7%), malignant tumors (20 cases: 26.7%) and others (19 cases: 25.3%).<br>6) Malignant tumors included 7 cases of lung cancer and the abnormal shadows were present on the chest X-ray films in 5 cases of them at the first visit to apply the designation. The histological types of them were various.<br>7) The expected death of designated patients was calculated based on the mortality of Mie-Prefecture. Observed deaths/Expected deaths was 1.48 in male and 1.88 in female.<br>8) In order to clarify the clinical features of the death cases due to respiratory diseases, a comparative study of death and alive cases was carried out.<br>The clinical features of death cases of bronchial asthma were older, higher age of onset and shorter duration of symptoms, more frequent episodes of pneumonia in past history and lower FEV1.0% than those of alive cases.<br>9) In the death cases of chronic bronchitis and pulmonary emphysema, the chracteristics of the clinical findings were larger amount of sputum and lower%VC and FEV1.0%in comparison with the surviving cases.
