Abstract

<p>Submucosal hematoma of the esophagus is a rare disease which occurs spontaneously and secondarily. Case 1 : An 81-year-old male with early stage gastric cancer underwent endoscopic submucosal dissection (ESD). When the ESD was completed and the endoscope was withdrawn, a submucosal hematoma was revealed in the middle esophagus. Proton pump inhibitor was administered in a fasting state. Esophageal endoscopy on post-operative day 8 showed reduction of the submucosal hematoma and a longitudinal ulcer. The ulcer was healed after treatment without stenosis. Case 2 : A 73-year-old male taking an antiplatelet agent for an earlier myocardial infarction and suffering from hepatitis C visited his family doctor complaining of epigastric pain after eating dinner. Endoscopic examination at the doctor's office revealed a submucosal hematoma from the middle esophagus to the cardia together with hemorrhaging. Rupture of the esophageal varices was suspected, and the patient was transferred to our hospital by ambulance. Emergency endoscopic examination revealed a submucosal hematoma and exfoliative mucosa, and a diagnosis of spontaneous submucosal hematoma was made. Treatment with proton pump inhibitor was initiated. The hematoma was absorbed and a shallow ulcer was formed in the area of the hematoma. The ulcer was healed a month after treatment. Here we report on these two cases of submucosal hematoma of the esophagus caused by different factors, and present a review of the related literature.</p>
