Aminopeptidase activity in various blood cells was determined and that in mature neutrophils of patients with blood diseases investigated, using fast blue B as the coloring reagent and L-leucyl-4-methoxy-β-naphthylamide hydrochloride as the substrate for histochemical demonstration and estimation of the enzyme activity.<br>With positive blood cells, the enzyme was demonstrable as intracytoplasmic crimson granules. Granulocytes, monocytes, reticulum cells, lymphocytes and thrombocytes proved to be positive for this enzyme while the erythrocyte series and plasma cell series were negative.<br>In healthy subjects, the mean aminopeptidase activity in mature neutrophils, expressed in terms of positivity index per 100 cells, was determined to be 252.0±43.1 and did not differ significantly between sexes.<br>The positivity index was normal or low in peroxidase-positive cases of acute leukemia and normal or elevated in peroxidase-negative cases. All patients with chronic myelogenous leukemia examined but one showed virtually normal values, and no significant alteration was observed following acute exacerbation. Nor was any conspicuous change observed following splenectomy in 5 cases of chronic myeloid leukemia. Cases of other hematologic disorders were also studied.
