抄録

我々は，EBV関連血球貪食症候群(EBV-HLH)加療中にtransfusion-related acute lung injury (TRALI)を発症した症例を経験したので報告する。症例は19歳女性。発熱，腹痛を主訴に来院し，汎血球減少，肝障害，DIC, 血漿中EBV-DNA高値，肝脾腫，骨髄中に血球貪食像と異型リンパ球を認めた。EBV-HLHと診断し新鮮凍結血漿を用いた血漿交換を行ったところ，開始2時間後より肺水腫による呼吸不全に陥ったが，ステロイドパルス療法及び非侵襲的陽圧換気にて救命し得た。輸血製剤から抗HLA class I抗体及び抗HLA class II抗体が検出され，患者Bリンパ球との交差試験も陽性であったため，TRALIと診断された。これまでTRALIを合併したEBV-HLHの報告はないが，HLHに伴う高サイトカイン血症がTRALI発症に関与した可能性も考えられた。
