A Case of Cheilitis Granulomatosa in an Infant

Description

A 2-year, 5-month-old girl with atopic dermatitis developed gingivitis, gum bleeding and a swollen upper lip after increasing consumption of dairy products. She underwent treatment at a dental clinic, but the upper lip swelling did not improve. She also had fissures, scales and xerosis on her elastic hard swollen upper lip and slight eczema around her lips. There was no facial palsy, fissured tongue, decayed tooth or metal dental work. Eliminating dairy products from the diet and treatment with anti-allergic agents and tranilast were ineffective. Histological examination revealed epithelioid cell granuloma, and she was diagnosed with cheilitis granulomatosa. Although treatment with oral corticosteroid showed some improvement, when the patient was tapered off the medication, the swelling of the upper lip slightly flared up again. However, after adding oral tranilast to the treatment, the swelling completely disappeared. We report this rare case with a review of the literature.Skin Research, 12: 339-342, 2013
