A Case of Giant Malignant Pilomatricoma

Description

The patient was a 51-year-old man. At presentation, he had an irregular, partly eroded, pedunculated tumor measuring 10×8×7 cm, with adherent calcified deposits, on the extensor aspect of the left arm. Magnetic resonance imaging revealed that the lesion was infiltrating into the fascia in some parts, while also involving the fatty layer. Biopsy of the tumor showed that it mainly consisted of basophilic cells and shadow cells. Nuclear atypia and mitotic figures were observed sporadically among the basophilic cells. A diagnosis of malignant pilomatricoma was made, and the patient underwent extended resection followed by chemotherapy. Because malignant pilomatricoma is particularly difficult to distinguish from benign disease in its early stage, many cases are only diagnosed after recurrence or metastasis. Even if this type of tumor is diagnosed as benign, it should be followed over the long-term. Moreover, lesions that grow rapidly or recur after simple resection will require extended resection.
