Combined therapy with surgery, radiotherapy, and chemotherapy for carcinoma of the lacrimal Sac and eyelid

Abstract

Since carcinomas of the ocular region are rare neoplasms, errors or delays in diagnosis are common. However, the carcinomas frequently metastasize to regional lymph nodes. We report four cases of carcinomas of the ocular region with regional lymph node metastasis. Two cases were sebaceous carcinoma of the eyelid, and the other two cases were mucoepidermoid carcinoma of the lacrimal sac and the eyelid. All four cases underwent wide surgical resection. Two cases with sebaceous carcinoma of the eyelid underwent local excision and the other two cases with mucoepidermoid carcinoma of the lacrimal sac and the eyelid underwent orbital exenteration. Total or superficial parotidectomy was performed in three cases. Neck dissection was performed in all the cases. The surgical defects were reconstructed with free flap transfer in three cases. Postoperative radiation therapy to the primary site, parotid gland and neck was performed in three cases. Only one case received preoperative chemotherapy. Three of four cases have been free of recurrent diseases. Only one case with mucoepidermoid carcinoma of the lacrimal sac died five months after the initial treatment. The chance of head and neck surgeons having to treat these patients in collaboration with ophthalmologists and plastic surgeons is expected to increase in future.
