Description

慢性閉塞性肺疾患（COPD）の病態把握にしばしば気流制限の程度が用いられる．しかし，気流制限の程度は，COPD患者の生活の質（QOL）や予後を反映すべき重症度と必ずしも一致しない．一方，患者の臨床症状や呼吸困難感およびQOLの程度は予後予測因子の一つとして知られる．COPD assessment test（CAT）は，①咳，②喀痰，③息苦しさ，④労作時息切れ，⑤日常生活，⑥外出への自信，⑦睡眠，⑧活力の8項目で患者のQOLを総合的に半定量でき，従来の質問票に比較して簡便で安価である．2011年度版のGlobal Strategy for Diagnosis, Management, and Prevention of COPDドキュメントでは，CATは，呼吸困難レベル，％1秒量や過去の増悪頻度と同様に，症状緩和あるいは将来リスク軽減を予測でき，治療戦略選択に考慮するように推奨されている．
