Carbohydrate antigen 19-9 (CA 19-9) and its associated blood group substance, Lewis antigen were studied in patients with biliary tract diseases. <BR>In biliary cancers, blood CA 19-9 were positive in 61 of 75 cases (81.3%), however, no superiority of CA 19-9 to the conventional biliary markers including ALP, LAP, and total bilirubin was shown. Biliary or cholecystic distribution pattern of Lewis A antigen, that has a relation with the antigen determinant of CA 19-9, was approximately similar to that of salivary Lewis manifesting type. In the course of cancer progress, a conversion of blood lewis type to Le (a-b-) was observed, however, salivary Lewis antigen should be referred to determine whether it was the true Le (a-b-) case or not.
