This paper presents a case of mediastinal schwannoma originating from the vagal nerve.<br> A 38-year-old woman was seen at the hospital because of dry cough and an abnormal shadow in a chest X-ray film which was pointed out at a medical checkup. Under a diagnosis of benign medistinal tumor, an operation was carried out. The tumor originated from the intrathoracic left vagal nerve and was resected by slicing the anterior side of nerve bundle attaching to the tumor without cutting off the nerve. The recurrent nerve palsy was not found after the operation, and histological diagnosis of schwanoma was made. Neurogenic tumors originating from the vagal nerve are rare. Twenty-seven cases which were given detailed explanations have been seen in the Japanese literature. Most mediastinal schwannomas arising from the vagal nerve are reportedly benign, but making the accurate diagnosis is often difficult preoperatively. The enucleation of the tumor under the capsule is suitable for the nerve preservation in case of schwannoma originating from the vagal nerve, but it is necessary to make accurate diagnosis before the enucleation.
