Abstract

Two cases of so called protein-losing gastric cancer which presents villous adenoma like morphorogy are reported. Both 71- and 86-year-old men were admitted to the hospital for close examination of edema, anemia, and hypoproteinemia. Upper GI series, endoscopy, CT, and ultrasonography examinations demonstrated a giant tumor in the stomach. Their serum total protein levels decreased to 4.0g/dl. 99mTc-HSA protein-losing test demonstrated the leakage of HSA to upper GI tract and gastric juice of both patients included very high level of protein (0.7g/dl and 2.2g/dl). They were diagnosed as so called protein-losing gastric cancer. Serum protein did not increase even after hyperalimentation and administration of albumin before surgery. Both patients underwent subtotal gastrectomy and lymph node dissection, and general condition and hypoproteinemia improved. Both tumors showed giant villous adenoma like form and califlower like form, and were in stage III and I, respectively. In the treatment of gastric cancer showing serious hypoproteinemia, we should to keep this disease in mind, and if this is suspected, early surgery is important without wasting time for improving serum protein level.
