Objective: It has been reported that the balance scores of patients wearing a soft orthotic device for the knee were significantly better than those of patients who did not wear such a device. Considering the effect of balance improvement in patients with the orthotic device, it is reasonable to test whether wearing the orthotic device on bilateral knees for patients with unilateral symptomatic osteoarthritis (OA) of the knee may augment the effect. <br>Methods: Fifty nine patients with knee OA were prospectively randomized and treated using soft orthotic devices for the knee in bilateral knees (the bilateral group) or the affected knee (the unilateral group) for 4 weeks. Each of the ten items included in the Lequesne Index of Severity were assessed at baseline and follow-up. <br>Results: Five patients withdrew from the study; the 54 patients who completed the 4-week study were evaluated. The bilateral group (n=27) demonstrated a significantly greater improvement in pain on walking than the unilateral group did (n=27) (P=0.028). For the other items in the index, there were no significant differences between the two groups (P > 0.05). <br>Conclusion: Patients with unilateral symptomatic knee OA who complain of severe knee pain on walking will show more improvement wearing bilateral soft orthotic devices for the knee than with unilateral use.
