抄録

Due to increase in population of the aged, there have been upward tendencies that the fractures of femoral neck occur frequently. Studies were carried out on 27 cases treated at the Shibukawa National Hospital from June 1, 1980 to May 31, 1982, with respect to such problems as types of fracture, operation methods depending on systemic conditions, post-treatment methods, complications etc.<BR>Ages of patients were from 54 & 89 years old, on the average of 70.4 years old; ratio of male to female was 5 : 22. This ratio reflected larger population of the aged female and further that among them post-menopausal bone atrophy and osteoporosis were apt to be considerably developed. Types of fracture were at the ratio of 6 intracapsular type to 21 peritrochanteric type. Most of injuries were resulted fror “falling”, in which 10 cases were of fractures of paralyzed side during treatment for hemiplegia due te cerebro vascular disorder in rehabilitation center or convalescing at home. From these circumstances, medical facility employees including attendants are required to pay careful attention to the patients.<BR>The primary objectives are to hold getting-up and sitting position and to ensure leaving-bed as earlier as posible. For intracapsular type fractures, usable methods are artificial femoral head replacement and total hip joint replacement with minimum operative stress kept in mind; and for the patients with severe anamnesis, cancellous screw and percutaneous multiple pinning methods are also effective. For the peritrochanteric type fractures, the Ender pin method seems to be effective for earlier rehabilitation.<BR>The prognoses seem to be poor in such cases as dementia, morale depression and athymia due to cerebro vascular disorder.
