甲状腺外科学会病理小委員会では甲状腺細胞診の報告様式を現在の甲状腺癌取扱い規約の様式から甲状腺細胞診ベセスダシステムへ移行することを検討中である。本稿では，取扱い規約とベセスダシステムの報告様式の違いを紹介すると共に，自験例の診断成績を用いて甲状腺細胞診ベセスダシステムの妥当性について検討した。取扱い規約とベセスダシステムの主な違いは，①細胞学的に鑑別困難な症例を，濾胞性腫瘍を疑う群（FN/SFN）とそうでない群（AUS/FLUS）の２つの診断カテゴリーに分けた点，②不適正の判定基準を明確にした点，③各診断カテゴリーについて悪性の危険度を数値で示した点の３点である。両者の基本的な考え方は類似しているので，大きな混乱なくベセスダシステムへの移行が可能と考えている。
