2005年に発刊された「科学的根拠に基づく急性胆管炎・胆嚢炎の診療ガイドライン」では，重症度に応じたタイミングで胆道ドレナージを行う診療指針が示され，重症，中等症の胆管炎に対しては緊急もしくは速やかに胆管ドレナージを行うことが推奨されている。今回我々の緊急ERCP（内視鏡的逆行性胆管膵管造影）130件の検討では，ガイドラインではまず保存的初期治療を行うとされる急性胆管炎軽症例が8件（6.2％），診断基準を満たしていないものが11件（8.5％）含まれていた。また，指導医の緊急ERCP参加率は69％で，非緊急例（88％）と比較し有意に低かった （P＜0.001）。ガイドラインの基準を満たさない例での緊急施行理由は，多くが休日前などで，初期治療に反応しなかった場合に時間外のスタッフ不足の状況でERCPを施行することを危惧したものであり，臨床の場ではその時の診療体制に応じて緊急ERCPの適応が判断されている現状があった。自験緊急ERCP例では非緊急例と比べ同等の手技成績と安全性をもって行えていたが，処置内容は緊急処置に携わるスタッフの経験値に左右される場合があり，ドレナージのみにとどまっているものが多かった。実際の臨床の場での緊急ERCPは必ずしもガイドラインに即していない場合もあり，各施設の状況を総合的に考慮して適応，処置内容を判断する必要があると思われた。
