- 症例 2種類のAH時間を示した稀有型上部房室結節内リエントリー性頻拍の1例

症例は頻拍発作を主訴とする38歳女性である.発作時心電図はnarrow QRSを示し,II,III,aVFで陰性のP波がQRS波の直前にある頻拍とT波の下降脚部にある頻拍との2種類が認められた.心房早期刺激で二重房室結節伝導路(DAVNPW)が認められたが,副伝導路の存在は否定された.室房伝導は認められなかったが,心室ペーシング,心室早期刺激により逆行性に房室結節の不顕伝導を生じ,洞性P波に対するAH時間が軽度延長するとAH時間が70-90msecの頻拍,AH時間が著明に延長するとAH時間が230msecの頻拍が誘発された.両頻拍ともcycle lengthは440msec,心房波の最早期興奮部位はヒス束部,P波の波形は同一で,VA時間は短くなかった.頻拍発作中に加えた心房早期刺激,および心室刺激,あるいは心室性期外収縮により前者から後者,後者から前者の頻拍への突然の移行が認められた.Verapamil静注が頻拍の停止および誘発予防に有効で,β-blocker内服により頻拍発作を予防できた.以上より両頻拍とも上部房室結節内のDAVNPWにおける稀有型房室結節リエントリー性頻拍で下位房室結節における伝導遅延の有無によりAH時間の異なる頻拍が出現したと考えられた.
