Description

運動誘発性心室頻拍(VT)を有する31歳,男性.イソプロテレノール(ISP)の投与下で,VT1(下方軸,I:QS,V1,V2:QS,移行帯V4)が誘発され,ATP急速静脈内投与により停止した.また,同頻拍中VT1とは異なるVT2(下方軸,I:QS,V1,V2:rS,移行帯V3)が出現し,VT1とVT2は互いに移行した.VT1の最早期興奮部位は右室流出路(RVOT)中隔側前下方と左室流出路(LVOT)側の右冠尖(RCC)で,VT2の最早期興奮部位はLVOT側RCCにあった.VT2の最早期興奮部位でのpacemapは高出力でVT1と,低出力でVT2と完全一致した.VT1に対してRVOTの最早期興奮部位で通電後,VT1およびVT2とも誘発不能となった.本頻拍はRVOT-RCC間の心室中隔に起源を有し,RVOTとLVOTの両側に興奮伝導するATP感受性特発性VTと考えられた.
