Description

A case of Wernicke encephalopathy following hyperalimentation without thiamine is reported. Retrocochlear deafness and such classic symptoms as opthalmoplegia and ataxic gait were observed. The MR findings were also considered to be useful for an early diagnosis of Wernicke encephalopathy. In response to the administration of thiamine both deafness and almost all of the ocular signs were observed to gradually improve. On the other hand, ataxia and lateral gaze nystagmus remained. The lesions observed in both vestibular nuclei may be attributable to the residual ataxia. A pure tone audiometer test showed almost normal hearing, while both speech discrimination tests showed poor scores. ABR revealed a slight prolongation of the unilateral III-V interwave intervals. Therefore, lesions around the inferior colliculus in the upper brainstem may account for this type of retrocochlear deafness. It is thus necessary to pay close attention to the onset of retrocochlear deafness as a potential sign of Wernicke encephalopathy.
