As a donor coordinator, a principal person in organ transplantation, the author had been working for 22 years, from 1995, the inauguration of Japan Kidney Transplant Network (currently, Japan Organ Transplant Network) to 2017. The author experienced nearly 500 cases of organ procurement mainly in Tokyo metropolitan area, including donation after cardiac or brain death, emergent carry of organs, and procurement support in adjoining areas. At the beginning, there was no education system of organ donation and procurement and I learned through my own experiences, i.e., trials and errors, under ambivalence and distress. When I think back, there was no convincing case but each had problems, more or less. Every time I tried to improve my skill by reviewing previous cases. Thus, my tutors were donor families, hospital staffs and persons who helped me. Having experiences of coordination, I learned the true meaning of transplant organ donation and to maximize donors’ will and families’ wish between the legal rule and emotion.
