Background: We report a case of immunoglobulin G4-related ophthalmic disease (IgG4-ROD) with recurrent optic neuritis without a prominent occupying lesion.<br>Case report: An 80-year-old man with a history of recurrent optic neuritis complained of poor vision in his right eye. His right eye had low critical fusion frequency, relative afferent pupillary defect, and reddish optic disc. Magnetic resonance imaging (MRI) revealed no occupying lesion in the orbit. Systemic corticosteroid therapy was effective for the subjective symptoms and objective findings. After 1 year, he presented with bilateral eyelid swelling. He had no low visual acuity, ocular movement restriction, and diplopia. However, his serum IgG4 level was significantly increased. MRI demonstrated bilateral lacrimal gland enlargement and high enhancement on a T2-weighted image, but no occupying lesion.<br>Conclusion: IgG4-ROD without a prominent orbital occupying lesion should be considered at the onset of recurrent optic neuritis.
