Description
Transjugular obliteration (TJO) has been performed for solitary gastric varix with splenorenal shunt. However, TJO can not be performed for gastric varix without splenorenal shunt. On the other hand, endoscopic variceal ligation (EVL) with Stiegmann's ligator can seldom be performed for solitary gastric varix, because the ligator is too small to ligate gastric varix.
Therefore, giant ligator with inner diameter of 10.3mm and length of 16mm was used to ligate gastric varix in 2 cases without shunt. Successive endoscopic examination showed gastric varix scarred off after ulceration. Gastric varix was not found endoscopically in 6 months and 4 months after EVL respectively.
In conclusion, it is thought that giant ligator could be one of the treatment for solitary gastric varix without shunt.
