When tumor cells are found in the peripheral blood in patients with malignant lymphomas, it is important to differentiate whether this is due to a leukemic state of malignant lymphoma or secondary leukemia associated with treatment for malignant lymphoma.<br>Development into a leukemic state of malignant lymphomas occurred often within one year from the onset, but the recent advance of treatment has brought an increase in numbers of long-survived cases.<br>We report a case of malignant lymphoma developed into a leukemic state after a complete remission for about seven years.<br>Case; A 73-year-old male. In December 1978, he was admitted to our hospital complaining of cough, dyspnea and swelling of left supraclavicular and right inguinal lymph nodes. Biopsy of the lymph nodes revealed the findings compatible with malignant lymphoma (diffuse, medium-sized cell type). VEMP-therapy led him to a complete remission.But after about seven years the tumor cells were found again in his peripheral blood. Flow-cytometric analysis of the tumor cells showed prethymic T-cell. He was diagnosed as a leukemic state of malignant lymphoma and was treated with VP-therapy. But he died from sepsis and disseminated intravascular coagulation after an admission for fifteen days.
