In this paper, we present a case of unresectable common bile ductal carcinoma treated with combined external irradiation and radiofrequency hyperthermia. A 72-year-old female with bile ductal carcinoma was referred to this hospital for palliative irradiation. At the point of the external dose of 2400 cGY, the expandable metallic stent was successfully placed at the stenotic site of the bile duct to maintain its patency. Thereafter, the intraluminal temperatures were accurately monitored without difficulty. So hyperthermia was safely administered for 40-60 minutes once a week for a total of four times combined with the cumulative external radiation dose of 5600 cGY. At the end of this treatment, the bile duct was almost completely re-canalized. So it was considered that this combination therapy effectively provided good palliation. However, it seemed likely that the external dose of 5600 cGy was too small to control this type of tumor because the patency of the bile duct was maintained for only three months. Therefore, it was considered that the intracavitary irradiation might contribute to deliver much more higher dose to the local tumor.
