- 肝に初発し術後１０年目ですいにも認められたカルチノイド腫ようの１剖検例

Description
症例は82歳男性. 主訴は吐血, 息切れ, 腹部CT・MRIにて膵体尾部に径約6cm大の腫瘤を認め同腫瘍は胃体上部後壁を圧排, 一部胃粘膜に突出して易出血性であった. 入院後低血糖発作を繰り返し約1年後に死亡した. 剖検で膵腫瘍はgrimelius染色, chromogranin染色陽性でありカルチノイド腫瘍と診断した. 本症例は10年前に肝腫瘍で手術(肝外側区部分切除術)を受けていたが, 再検にて肝の腫瘍もカルチノイド腫瘍と判明した. 肝, 膵のカルチノイド腫瘍は極めてまれであり, 低血糖の病態も含め報告する.
