Plasma levels of catecholamine(CA) and c-AMP were studied in patients with liver disease in an attempt to evaluate the role of autonomic nervous as well as vascular system in liver disease. The patients consisted of the following: 22 cases of liver cirrhosis, 15 cases of chronic hepatitis, and 20 cases of acute hepatitis. CA was carried out by automatic rapid liquid chromatography and c-AMP was determined using CPBA method. Normal plasma values of epinephrine(EP), norepinephrine (NE) and c-AMP were as follows; below 0.12ng/ml, 0.04-0.35ng/ml and 8.3-13.5p molelml, respectively. CA values revealed that no elevation of EP was found in all patients examined; whereas a rise in NE was solely seen in patients with liver cirrhosis, showing in 17 out of 22 (77.3%) elevated and the value was 0.55±0.37ng/ml (mean±S.D.) (p<0.01). On the other hand, c-AMP indicated comparatively high in the patients with acute hepatitis (20.3±8.3) and in those with liver cirrhosis (24.2±8.9), contrasting with the lowered levels in those with chronic hepatitis (13.9±3.3) (p<0.01). In comparing these data with other laboratory findings in the patients with liver cirrhosis, NE had a correlation with the serum levels levels of gammaglo-bulin (r=0.6514, t=3.84>2.90). Furthermore, hepatic angiography showed marked cork screw appearance in the cases with elevated NE values.<br>The data obtained lend strong support to the view that derangement of sympathetic nervous system with resultant disorder of vasoactive substances are involved in chronic liver disease, and c-AMP may be responsible for the repair of wound tissue.
