Abstract

針肝生検に応用可能な原発性胆汁性肝硬変（PBC）の新しい病期分類，活動度分類を提唱した．病期分類は，サンプリングエラーを少なくするため，肝の線維化と胆管消失を中心に，病変をスコア化し，stage 1～4に分類した．活動度分類として，胆管炎cholangitis（CA）の程度をCA0～3に，また肝炎hepatitis（HA）の程度をHA0～3に分類した．今回の活動度分類・病期分類によりPBC患者の病態，病勢が半定量的に評価でき，オーバーラップ症例の診断や薬物治療の選択および効果判定にも有用と考えられる．今後，本活動度分類，病期分類を用いた評価が臨床，研究の分野で展開することを期待している．<br>
