- 大腸ESDの適応と実際―現状と今後の展望―

Description
大腸ESDは，2012年4月に「早期悪性腫瘍大腸粘膜下層剥離術」として，早期大腸癌を適用として保険収載されたが，機器の開発・改良や手技の工夫によりその技術的ハードルは徐々に低くなっている．大腸ESDの適応基準は「一括切除が必須であるがスネアによる一括切除が困難な腫瘍性病変」であり，大腸ESD標準化検討部会より提唱された適応が広く受け入れられている．現時点における大腸ESDの主な手技的課題は，高度線維化合併とスコープ操作性不良例への対策である．将来的に大腸T1（SM）癌に対する完全摘除生検手技としての役割が大腸ESDに期待される．
