<p>切除不能な遠隔転移を有するStage IV大腸癌の治療において原発巣の切除およびそのタイミングについてはさまざまな考え方がある．特に無症状な原発巣の切除の意義は定まっていない．原発巣切除が予後延長に寄与する可能性が示唆されているが，明確なエビデンスはない．進歩した化学療法により原発巣制御が可能であり切除は不要とする報告もある．原発巣切除の適応は原発巣および遠隔転移巣の状態や全身状態，予想される予後と症状，手術リスクを検討して判断する．現在，無症状な原発巣を有する切除不能Stage IVに対する姑息的な原発巣切除の意義を検証するランダム化比較試験が進行中である．</p>
