Abstract

症例は70歳,男性.胃腸炎症状を契機にネフローゼ症候群を呈し入院となった.下腿に紫斑を認め,血清補体価低値,リウマチ因子高値,クリオグロブリン(IgMκ型M蛋白とIgGの混合型)陽性であった.腎生検では膜性増殖性糸球体腎炎(MPGN)像を呈し,電顕にて内皮下に微細な管状構造を呈する多量の沈着物を認めた.クリオグロブリン血症に伴うMPGNと診断し,プレドニゾロン(PSL) 40mg/日, cryofiltrationにて加療を行ったが腎機能は改善せず.ステロイドによる精神症状,肺結核を合併したためPSLを中止し,維持血液透析を行った.
